Complete our online referral form through the portal below to begin the straightforward process of medical verification for your shotgun/firearms licence application or renewal.

The fee for your firearms/shotgun medical report is £75, and we aim to complete your report and submit it to you and/or your designated firearms officer (depending on the police force) within 3 working days of receiving your full medical records from your GP surgery.

N.B. rules state your GP must release the records within 28 days of request, this can often be breached and we ask for your patience in this event.

1 Complete the referral form

Once we receive the referral form, we will be able to request a copy of your medical records from birth. Your General Practice are then given 30 days to send us your medical records.

2 GP reviews your medical records

We will then allocate your case to our GPs who will review your records and complete your proforma.

3 Report sent to candidate or police force

Once we receive your proforma, we will send it back to you and/or to the firearms licensing officer depending on the force requirements.
